Schistosomiasis or blood fluke disease, also known as Bilharziasis, is a parasitic disease caused by helminths from a genus of trematodes entitled Schistosoma. It is a snail-borne trematode infection. The disease is among the Neglected Tropical Diseases, catalogued by the Global Plan to combat Neglected Tropical Diseases, 2008-2015 and is considered by the World Health Organization (WHO) to be the second most socioeconomically devastating parasitic disease, next to malaria. WHO demonstrates that schistosomiasis affects at least 200 million people worldwide, more than 700 million people live in endemic areas, and more than 200.000 deaths are reported annually. It leads to the loss of about 4.5 million disability-adjusted life years (DALYs).

Man is infected by working in infested water containing living cercariae. The cercariae penetrate the skin or mucous membrane, develop into schistosomules and travel through the peripheral venules to the pulmonary vessels, from where they enter the general circulation. Those reaching the portal venous system develop into male and female adult worms in 6-12 weeks; most adult S. mansoni then migrate to their final habitat in the tributaries of the inferior mesenteric veins. Most eggs laid in the small venules around the colon pass into the tissues, but some are eliminated in the stools. Eggs which are passed in the stools hatch immediately in fresh water and liberate free- swimming miracidia, that penetrate a specific snail host (genus Biomphalaria) within 24 hours. Cercariae developed in 4-6 weeks and are passed into the water where they remain infective for 2-3 days. The cycle is then repeated when man enters fresh water containing the cercariae and becomes infected.

The epidemiology of schistosomiasis is well documented and its geographic distribution has been mapped and there is an ongoing mapping in Ethiopia. Nevertheless, new transmission foci have been discovered in different parts of the country. The objective of this study was to assess the establishment of transmission and determine the prevalence of S. mansoni infection in school children from Sanja Town, northwest Ethiopia. In this study, Stool specimens were collected and microscopically examined using Kato-Katz and SAF concentration methods. Malacological survey was also carried out to identify snail intermediate hosts and larval infection rate in the snail. The snails collected were checked for trematode infection by shedding. The prevalence of Schistosomiasis mansoni observed in the present study was very high (82.8%). The prevalence of Schistosome infection in Biomphalaria pfeifferi was also high (16.9% and 0.027% during February and April, respectively). The study has shown establishment of transmission of Schistosomiasis mansoni in Sanja Town. Therefore, appropriate integrated control measures need to be introduced to reduce morbidity in the population.

The aim of this study was to determine whether Schistosoma mansoni is a health problem in Kigungu fishing village in Entebbe, Uganda. In addition, the study was also meant to find, as in other S. mansoni endemic areas how antibody isotypes responses against S. mansoni soluble egg antigens (SEA) and soluble adult worms protein (SWAP) vary with host sex, age and infection intensity. To do this I carried out a cross-sectional cohort study for Schistosoma mansoni among Kigungu residents in which one thousand participants reported for screening and enrolment. Nine hundred and forty three volunteers were recruited and microscopic stool examination revealed that, before treatment, (47.5%) were S. mansoni positive. Among these, (56.5%) were males and (43.5%) were females. The age of study patients ranged from 5 to 60 years, (mean age was 19 years, and median 33.5 years). Eighteen months after praziquantel treatment 625 (66.3%) out 943 participants came back for follow up study and they were reviewed for re-infections and antibody levels. The microscopic stool examination showed (39.4%) were still positive for S. mansoni. More males, (58.5%) than females (41.5%) were re-infected. Infecti
